The role of the community pharmacist in cardiovascular disease management

- Description: Cardiovascular disease contributes to mortality and morbidity statistics worldwide and in South Africa. The current focus in health care revolves around activities aimed at preventing the development of cardiovascular disease, rather than the treatment of disease. The identification of risk factors that can predispose a patient to the development of cardiovascular disease is an essential component of any cardiovascular disease management programme. It is necessary that in the management of these risk factors, they are not considered to be isolated, but inter-related. Through the provision of point-of-care cardiovascular risk screening and monitoring services as well as disease-related counselling, the community pharmacist, as a readily accessible source of healthcare, can play an essential role in the cardiovascular disease management process. The aim of this study was to describe the nature of the services provided by community pharmacists with respect to cardiovascular risk and disease management in the Nelson Mandela Metropole. The research design was a non-experimental, descriptive study using a crosssectional survey method. Data was obtained through the utilisation of a questionnaire. The questionnaire consisted of three sections and was administered to community pharmacies in the Nelson Mandela Metropole, that provided cardiovascular point-of-care screening services. The community pharmacists correctly identified cardiovascular risk factors such as obesity (76.6 percent; 36, n=47) and smoking (27.7 percent; 13, n=47). Other cardiovascular risk factors such as abdominal obesity (4.2 percent; 2, n=47), gender (2.1 percent; 1, n=47) and family history (4.2 percent; 2, n=47) were largely ignored by the pharmacists. Point-of-care testing services were readily available in the pharmacies, with all of the pharmacies providing blood glucose and blood pressure measurements. Blood cholesterol measurements were only provided in 87.8 percent (36, n=41) of the pharmacies. The services were generally provided in a clinic facility, with 90.2 percent (37, n=41) of the pharmacies having a clinic facility available. Pharmacists were involved in the provision of point-of-care services, with 85.4 percent (35, n=41) of the pharmacies indicating that the pharmacists participated. Pharmacists readily provided counselling prior (70.7 percent; 29, n=41) to and after (80.5 percent; 33, n=41) the conduction of the screening services on areas such as lifestyle modification and treatment options. Only 15 percent (7, n=47) of the pharmacists indicated that they were aware of Cardiovascular Risk Calculator Tools and none of the pharmacists indicated that they had utilised such a tool. Pharmacists recommended frequent monitoring (60.5 percent; 26, n=43) and lifestyle modification (67.4 percent; 29, n=43) to patients, if the result of their screening service was within normal limits. However, the majority of the pharmacists indicated that they would refer patients, if the results obtained were out of the normal range. Conclusions based on the findings indicated that the pharmacists are readily providing cardiovascular risk screening services. The pharmacists were also able to identify the presence of any risk factors that can lead to the development of cardiovascular disease in the patients. However, active pharmaceutical involvement in further cardiovascular disease monitoring seemed to be lacking. Recommendations were made on areas such as reimbursement for pharmaceutical care services, increased utilisation of support staff and Continuing Professional Development events that could assist in improving the role of the community pharmacist in cardiovascular disease management.

The provision of good antenatal services ensures early detection and prompt management of any complication or disease that may adversely affect pregnancy outcome. In order to ensure high quality care, antenatal services need to be evaluated at regular intervals, both from provider and client perspective, to ensure their effectiveness in improving the health status of pregnant women. The midwife, as the first contact person for most pregnant women attending antenatal clinics in South Africa, has the potential to play a major role in improving the health status of these women. However, for the midwife to be effective in achieving this, antenatal services need to be effectively utilized by women. One of the factors that affect utilization of any service is client satisfaction with the service being rendered. This study, therefore, explores the perceptions that pregnant women have of the care that they receive from midwives at the selected antenatal clinics. The objectives of the study are to: {u100083} Explore and describe the perceptions and experiences of pregnant women attending antenatal clinic regarding the care they receive from midwives. {u100083} Make recommendations to assist registered midwives in optimizing the accompaniment of women during the antenatal period. A qualitative, descriptive, exploratory and contextual design was used for the study. The sample was chosen from the target population by means of purposive sampling and data was collected through unstructured interviews with the participants. Before data collection, permission was obtained from the Eastern Cape Department of Health and the Nursing Service Manager of the Gateway clinic, in the district hospital where the research was conducted. The Nursing Service Manager was acting as a Medical Superintendent at the time of the study. Written, informed consent was obtained from all participants before conducting interviews. Trustworthiness was ensured by means of Guba’s model throughout the study, and the aspects of truth value, applicability, consistency and neutrality were considered. Tesch’s eight steps of data analysis were used to analyze the data collected, and four main themes were identified namely: • Women perceive midwives as considerate and knowledgeable • Women perceive midwives as lazy and rude • Women experience mixed emotions about the care they receive from midwives • Though their experiences, women identified certain needs in the services and care they received at the clinic. Conclusions were drawn and recommendations for midwifery practice made based on the results of the study, with the aim of improving antenatal services rendered to pregnant women.

A journey with an abused child : a non-directive play therapy perspective

- Description: The extensive amount of research conducted in the field of family violence internationally indicates that child abuse has a detrimental effect on both the physical and emotional development of children as well as having a profound effect on an individual’s psychological development and functioning in adult life. The aim of this research study was to describe the therapeutic process that unfolded with a seven year old allegedly abused female client within the framework of non-directive play therapy. The case was further contextualised utilising Erik Erikson’s theory of psychosocial development. This study served to broaden the body of psychotherapeutic knowledge by means of meaningful qualitative enquiry. The client was referred for therapy because of severe emotional and behavioural problems following the alleged sexual abuse. The client was seen over a period of eight months and this included 11 sessions of non-directive play therapy, three parent interviews and psychometric assessments conducted by a colleague. The case study method was utilised in this study. To achieve the aim of the research, the methodology of choice was the descriptive dialogic case study. A purposive sampling technique was used in the selection of the research subject for this study. The data collection and analysis were conducted according to Yin’s (1994) analytical generalisation, which consists of two main strategies: (a) using a theoretical framework as a guide to determine what data is relevant; and (b) developing a matrix as a descriptive framework for organising and integrating the data. Furthermore, the process of data analysis was aided by the use of guidelines proposed by Irving Alexander (1988) with Axline’s non-directive play therapy and Erikson’s theory of psychosocial development as the theoretical frameworks. The findings of this study suggest that plotting the play therapy sessions according to the framework of the four stages of play therapy was a particularly useful tool to monitor Michelle’s progression through the therapeutic process. This can be seen as a valuable application of a tool which can be used within the non-directive play therapy approach. Contextualising Michelle’s development according to the stages of Erikson’s theory of psychosocial development was also found to be a valuable endeavour. According to her chronological age, Michelle should have been in the fourth stage of industry versus inferiority, but in reality Michelle was still struggling to strike a healthy balance between the terms of conflict of trust versus mistrust issues of the first stage. From a therapeutic point, this was an important exercise as it helped to inform the therapist and consequently, the therapeutic process. This research undertaking can be recognised as a positive demonstration of the value of non-directive play therapy (Virginia Axline) and Erik Erikson’s theory of psychosocial development in the therapeutic process. In addition, this study has served to facilitate a more holistic understanding of the case study approach to research. Recommendations regarding future research undertakings that utilise the case study approach and methodology have been made. - Description: While the field of career psychology has shifted its attention to the study of career development, the developmental stage of childhood has often been neglected. Similarly, there is an absence of career research of a longitudinal nature, despite consistent calls to focus more on longitudinal methods when studying career development. The present study forms part of an existing longitudinal project which investigates the career development of South African children. This study, initiated in 1998, attempts to explore the occupational aspirations and gender stereotypes of a group of 39 children in the Nelson Mandela Metropole. The results from this study will provide much needed baseline information on how South African children become aware of careers, as well as provide useful suggestions on how to develop career education syllabi, particularly within the Life Orientation curriculum. The present study aims to explore and describe the changes that may occur in the participants’ occupational aspiration interest typology, status levels, and gender stereotypes over a five year period. Both developmental and career developmental theories were used to provide a theoretical context from which the participants’ career development could be explored. The study is quantitative in nature as it made use of semi-structured interviews and a biographical questionnaire in order to quantitatively transcribe the data. The questionnaire consisted of four broad questions which asked for information regarding the participants’ occupational aspirations, how much information the participants had about the chosen occupation, the number of occupations the participants knew about, and the extent to which the participants held gender stereotypes regarding fourteen different occupations. Descriptive statistics were used to describe the findings of the data which had been coded according to Holland’s typology of occupations. In addition, content analysis was performed to elicit themes regarding the participants’ own reflections on their career development. Results from the data analysis revealed that the majority of participants aspired to Social type occupations throughout the five years under study. Furthermore, most participants consistently aspired to high status occupations throughout the five years. Similar results were found for girls and boys. The results also demonstrated that the participants’ gender stereotypes tended to decrease over time, particularly as their occupational information increased. Lastly, most children were able to reflect on their career development and attributed changes in their occupational aspirations to changes in their interests. The results from this study offer insight into the development of occupational aspirations and perceptions of South African children which can be used as a foundation for future research and which should be valuable in the development of relevant career education programmes for South African schools.

- Description: Despite the acknowledgement of the developmental nature of careers, little attention has been focused on identifying shaping influences during childhood. This noticeable deficiency in the literature marked the start of the current longitudinal study which was designed to provide information on the career development of nine to twelve-year old South African children. In South Africa research focusing on preadolescent career development is limited with existing research indicating the need to further explore how children’s career choice process initially develops. The study aimed to explore and describe the changes that occur, if any, over a four-year period in terms of occupational interest typology, occupational status levels, occupational gender stereotypes and occupational gender traditionality. Both developmental and career developmental theories were used to provide a context for exploring this process of preadolescent career development. It was decided that quantitative research methods would provide the necessary structure needed to conduct the research. Although semistructured interviews and biographical questionnaires were used to record the verbal data, the data obtained was transposed into nominal data for analysis. Participants responded to questions within a semi-structured interview that focused on four broad areas of questioning. These questions explored participants’ occupational aspirations, how many occupations they knew about, how much they knew about the identified occupation, and the extent to which they held gender-role stereotypes about certain occupations. After the data was iv Key words: career development, children, longitudinal research, occupational aspirations captured it was coded according to Holland's (1985) classification system of occupational interest types. The occupational aspirations expressed by the participants were also coded in terms of their status levels. The coded data was then further analysed using descriptive statistics in the form of frequency counts and percentages. Results from the study support both child and career development theory which view childhood as a time of rapid growth with critical development in various competency areas (including occupational aspirations and perceptions). Although the current study found that most children aspired to Social occupations, the popularity of this typology decreased with increasing age as more children aspired to Investigative, Artistic, and to a lesser extent, Realistic typologies at age twelve. Furthermore, it appears that children, like adults, are aware of status differentials among occupations and that this awareness subsequently influences occupational aspirations. - Description: Despite the acknowledgement of the developmental nature of careers, little attention has been focused on identifying shaping influences during childhood. This noticeable deficiency in the literature marked the start of the current longitudinal study which was designed to provide information on the career development of nine to twelve-year old South African children. In South Africa research focusing on preadolescent career development is limited with existing research indicating the need to further explore how children’s career choice process initially develops. The study aimed to explore and describe the changes that occur, if any, over a four-year period in terms of occupational interest typology, occupational status levels, occupational gender stereotypes and occupational gender traditionality. Both developmental and career developmental theories were used to provide a context for exploring this process of preadolescent career development. It was decided that quantitative research methods would provide the necessary structure needed to conduct the research. Although semistructured interviews and biographical questionnaires were used to record the verbal data, the data obtained was transposed into nominal data for analysis. Participants responded to questions within a semi-structured interview that focused on four broad areas of questioning. These questions explored participants’ occupational aspirations, how many occupations they knew about, how much they knew about the identified occupation, and the extent to which they held gender-role stereotypes about certain occupations. The occupational aspirations expressed by the participants were also coded in terms of their status levels. The coded data was then further analysed using descriptive statistics in the form of frequency counts and percentages. Results from the study support both child and career development theory which view childhood as a time of rapid growth with critical development in various competency areas (including occupational aspirations and perceptions). Although the current study found that most children aspired to Social occupations, the popularity of this typology decreased with increasing age as more children aspired to Investigative, Artistic, and to a lesser extent, Realistic typologies at age twelve. Furthermore, it appears that children, like adults, are aware of status differentials among occupations and that this awareness subsequently influences occupational aspirations. Consequently, this study aimed to qualitatively explore the item content of the South African adaptation of the WAIS-III in order to identify items that might still be potentially biased with specific reference to cultural and linguistic issues. The researcher employed purposive sampling, a non-probability technique, to select psychology professionals to act as expert reviewers. The sample consisted of 20 registered psychologists, psychology lecturers and psychology interns from the predominant culture and language groups of the Eastern and Western Cape. Qualitative and quantitative data were collected by means of a Bias Review Questionnaire which consisted of fixed-choice questions, as well as sections in which specific comments could be indicated. The questionnaire was distributed to 20 selected participants who were qualified to administer psychometric measures and were registered with the Health Professionals Council of South Africa (HPCSA). Qualitative data was analysed using Tesch’s model of thematic content analysis, and five main themes relating to potential bias were identified, namely: Culture, Language, Education, Socio-economic status, and Format. The qualitative data was re-submitted to the expert review group for approval and further comment. Quantitative data obtained from the Bias Review Questionnaire was analysed by obtaining frequency counts, which were converted to percentages, per subtest and aspect reviewed. The major findings of the study were as follows: 1. Cultural bias is evident in the South African adaptation of the WAIS-III. The differential meanings ascribed to concepts, terms, phrases and words by diverse cultures impact on test performance, as does familiarity with stimulus materials used in the measure. 2. Language bias was detected against groups with English as their second or third language owing to the unfamiliar, and sometimes archaic, terms and vocabulary used in the measure. The intended meanings of the translated items are unclear and confusing for test-takers. 3. South African test-takers have experienced vastly divergent educational opportunities which impact on intelligence test performance. The measure contains potential bias against individuals from disadvantaged educational backgrounds. 4. The content of the items is not relevant to test-takers from deprived socioeconomic backgrounds that have been unable to access good-quality education. 5. Format bias was found to exist with respect to the timed tests. Cultural groups who do not value speededness are disadvantaged by the structure and format of the measure. Based on the findings of this study, certain recommendations were made. These include: 1. Additional qualitative investigation of the WAIS-III , with a view to further adaptation in an effort to eradicate bias on cultural or linguistic grounds, and 2. Conducting further Differential Item Functioning (DIF) analyses and establishing construct equivalence across cultural and linguistic groups to promote this objective. Among the contributions believed to have been made by this study are the identification of potentially biased subtests and items on the grounds of culture and language, which will facilitate possible further revision and adaptation of the measure. - Description: It is estimated that the failure rate of small, medium and micro enterprises in South Africa’s (SMME’s) are between 70 percent and 80 percent. As a result, millions of rand are lost on business ventures because of essentially avoidable mistakes and problems. Difficulties encountered by small business owner-managers can be described as environmental, marketing, financial or managerial in nature. The primary objective of this study was to investigate the nature and importance of problems experience by small business owner-managers in the private health sector. The design that was used in this study was qualitative, descriptive, exploratory and contextual in nature. This study fell within the broad framework of qualitative research, as interviews were employed to do the research project (Baker, 1999:247). A purposive sample was used to identify the participants for inclusion. The target population was small business owner-managers operating within the private health sector in the Nelson Mandela Metropole. The sample comprised eight business owner-managers in the health sector within the boundaries of the Nelson Mandela Metropole, purposely identified. Before doing the empirical research, a pilot study was conducted with one small business owner-manager that fulfilled the required criteria of the research population. Data was collected by means of naïve sketches and semi-structured interviews. Data was analyzed according to the framework provided by Tesch (in Creswell, 1994). Guba’s model (in Krefting, 1991) was employed for data verification. Interviews were conducted in both Afrikaans and English. iii Direct transcriptions from the tape-recorded interviews ensured that all data was gathered and saved for reference when needed during the data analysis as described by Field and Morse (1996:64). The researcher took observational, theoretical, methodological and personal notes as described in Wilson (1989: 433-435). Transcriptions from sketches and interviews served as the database for the study. Only literature pertaining to the research process was consulted prior to the data collection of the primary data, in order to establish an effective research format. The problems experienced by small business owner-managers in the health sector were discussed under four major themes and several sub-themes identified by the researcher and the independent coder. Improving institutional responsiveness and focusing on Science, Technology, and Engineering and increasing the number of university graduates are amongst the more prominent strategies for changing the educational, socioeconomic, and political landscape within a global context. This research, as the first cycle of an action research project, explores collaborative group work with a group of science students at a Vista University campus (that is now part of the Nelson Mandela Metropolitan University) as a contribution to institutional, professional, and personal responsiveness. The treatise traces my development as a novice researcher within an evolving action research context that became a terrain for facilitating a collaborative approach to learning. I describe my personal experience and the experiences of my co-researchers as collaborative partners, the systemic influences considered during the study, and the process of action research that encouraged movement from feelings of apprehension and inadequacy to feelings of anticipation and excitement regarding collaborative interactive learning and development opportunities. For the co-researchers and me an action research process in an interpretivist paradigm was not just suited to an exploration of collaboration, but also evolved into a vehicle for interactive teaching and learning, in a collaborative and student-centred way. Giving voice and being listened to, having perspectives validated, engaging in learning that could accompany academic and personal growth, and an acute sense of being empowered are ingredients that participants, and institutions of learning, can continue building on and building with along evolving spirals of life-long learning and meaning making.

- Description: The primary aim of this study was to investigate the seasonal changes that occur in the anthropometric, physical and motor characteristics of elite junior soccer players over the course of a training year. The participants targeted were elite soccer players from the NMMU-FCK Soccer School of Excellence and a total of 18 players between the ages of 15 to 18 years were recruited to comprise the sample for the study. The participants in the sample were assessed on four separate occasions. Each testing period was separated by a three month interval with the first testing period taking place in June 2005, at the start of their pre-season period, and the last testing session taking place in March of the following year, at the end of their playing season. In order to determine which specific characteristics of soccer were to be tested, the relevant literature was reviewed to establish the requirements, attributes and indicators of success in elite soccer. Therefore the test battery used was compiled in an attempt to address each of the identified physical attributes that contributes to successful soccer play. The battery included the following tests: weight, height and percentage body fat to represent the anthropometric characteristics. The physical characteristics were assessed by testing flexibility, anaerobic speed endurance, aerobic endurance, and muscular strength tests (1 RM bench and leg press, grip strength, and isokinetic knee strength). The motor assessments comprised of tests for speed (10-m, 20-m, and 40-m), explosive leg power (vertical jump), agility, and soccer-specific tests (dribbling and shooting at goal). In addition maturity status during the study was assessed using grip strength and height scores. All of the participants within the study sample were assessed on each day of the testing period, in alphabetical order, and this order was maintained during each testing period. Each of the four testing sessions lasted three days. On entering the Nelson Mandela Metropolitan University-Biokinetics and Sport Science Centre (NMMU-BSSC) laboratory on day 1, the participants completed the test battery in the following order: anthropometric measurements (weight, height, skinfolds), flexibility test (sit and reach), leg power test (vertical jump), muscular strength tests (1RM bench press, 1RM leg press, grip strength), and an aerobic endurance test (Multi-Stage Shuttle Run). The tests conducted on day 2 of the test battery were conducted as field tests at the NMMU-FCK soccer playing fields and the participants completed the tests in the same alphabetical order as day 1. The testing battery was conducted in the following order: speed tests (10m, 20m, and 40m sprinting), agility test (Illinois agility run), soccer-specific tests (dribbling and accuracy tests), speed endurance test (Yo-Yo intermittent recovery test). Testing on day 3 consisted of Isokinetic dynamometry strength testing and was conducted at the NMMU-BSSC. Each participant completed the testing protocol according to the same alphabetical order as was used on days 1 and 2. In order to achieve the aim of the study, the following objectives were set: to determine the anthropometric, physical and motor characteristics of the participants at the pre-season, start of season, mid-season and end of season stages of the training year and to identify the peak scores achieved for each variable tested; to compare the variables tested at each of the four testing sessions to determine the progression of each variable tested and to identify at which testing session peak performance related fitness was achieved; to compare the results obtained with relevant data from other research literature; and to monitor the maturation status of the participants. Based on the results achieved it can be concluded that the participants achieved peak physical fitness at the mid-season period (testing session 3). It would appear that the attainment of these results was determined by the participants being exposed to both a pre-season and an in-season training period. It seems that both the completion of the specific types of training that occurred during these two periods, as well as the competitive matches played, elevated the participants physical fitness to the levels required for success at the elite level.

- Description: The apparent increase in the incidence of both anorexia nervosa and bulimia nervosa worldwide has resulted in a surge of interest in effective treatment, prevention programmes and health promotion. Health promotion and the primary prevention of eating and body image problems among young people, and in particular adolescents, is emerging as one of the most desirable achievements in contemporary health and nutrition education. Eating disorders usually have their origin during the teenage years, and as such, high schools provide useful sites for the implementation of prevention programmes. Educators can play an important role in the prevention of eating disorders and act as socialization agents who either reinforce or buffer the dominant societal discourses that shape young women’s views of themselves. There are calls, however, for caution in the design and implementation of school-based eating disorder curricula as school educators may inadvertently do more harm than good. It has also been suggested that female educators, as other women, are likely to possess a degree of normative discontent with their body shape and size, and that this dissatisfaction and negative beliefs about food may be unknowingly transferred to the learners within their care. The current study used an exploratory, descriptive research design to investigate the knowledge, attitudes and behaviours related to eating disorders, body image and weight control of a group of Life Orientation educators. A biographical questionnaire, a questionnaire designed for the purposes of the current research and two standardised paper-and-pencil questionnaires, namely the Body Shape Questionnaire (BSQ) and the Eating Attitudes Test (EAT), were administered to 50 female Life Orientation educators in the Nelson Mandela Metropole. A non-probability purposive sampling technique was used in the selection of participants and descriptive statistics were used to explore and describe the data. The results of the current research study indicated a lack of knowledge in those Life Orientation educators assessed regarding eating disorders and healthy diet. The results also indicated inaccurate knowledge amongst those educators assessed regarding effective and safe teaching practices of eating disorder pathology. The results of the two standardised questionnaires reflected an internalisation of the dominant societal ideals regarding weight and body shape, with 18% of the sample xi demonstrating attitudes and behaviours that could be indicative of eating disorder pathology of either clinical or subclinical proportions. Suggestions were made regarding future research and the need for further training of Life Orientation educators. Finally, the limitations as well as the value of the research were outlined.

In the OPD patients are assessed and treated for all chronic conditions including DM, and patients are seen at monthly intervals or when necessary. The OPD serves the neighbouring black population from the surrounding townships as well as the informal settlements. The effects of urbanisation have resulted in this area having a semi–rural, semi-urban population that is mostly Xhosa speaking. The bulk of the population is unemployed, while others are living on a minimal income. Unemployment in the Eastern Cape ranges from 40% in rural areas, rising to 50 - 60% in the urban areas (Proposed aluminium Pechiney smelter within the Coega IDZ, 2002:4-8). In some homes there is no or little money to buy food, and even less to make use of a health service. Some people live on either a social grant for the elderly, a grant for young children or a disability grant and most people in this area have an income below the level at which payment of taxes for contribution to the economy is possible.

- Description: The subject of mood disorders and in particular depression is pertinent with rapidly increasing incidences of depression and suicide a widespread phenomenon in the world today. In South Africa, the rates of depression are increasing steadily each year. Much research has been undertaken in the area of depression, with negative cognitive schema identified as a common factor, which increases an individual’s vulnerability or diathesis to depression and hopelessness. The primary aims of this research are to explore and describe the depressogenic cognitive schema of both a depressed as well as normal (nondepressed) individuals and identify the relationship these schema have to levels of depression and hopelessness. In order to achieve these objectives, three measures were administered, the Beck’s Depression Inventory, the Beck’s Hopelessness Scale and the Inferential Style Questionnaire. The research design is quantitative in nature and took the form of an exploratory-descriptive study. The researcher made use of frequency tests to identify frequencies of scores and descriptive statistics to identify the mean, range and standard deviations. T tests, a Pearson product- moment correlation coefficient and factorial analysis of variance (ANOVA) were also employed for statistical analysis within this research study. The findings of this research study identify that the depressed sample scored higher levels of depression, hopelessness and negative inferential style than that of the normal sample.

Public sector pharmacists' perception of the public sector performance management system

- Description: Performance Management aims to develop the employee and ensure that the work which the employee does is in line with organisational goals. However, many managers and employees do not like performance management systems and very often, for this reason, such a system does not succeed in meeting the organisation’s goals. The aim of this study was to determine the perceptions towards the performance management system of pharmacists working in the public sector. A questionnaire was compiled and pharmacists working in all public sector hospitals, provincial and municipal clinics and medical depots were asked to complete the questionnaire. The response rate was 66 percent. Seventy three percent of respondents had never undergone a performance evaluation, 75 percent stated it did not motivate them, 62.5 percent felt it did not improve poor performance, 90.6 percent felt the Performance Appraisal System did not reward good performance sufficiently and 63.6 percent felt it did not help with career progression. Seventy eight percent believed that the Performance Appraisal System did not effectively measure the pharmacists’ performance, and 82 percent felt that the System needed to be developed further. At least four evaluations should have been completed per year, yet 85 percent of respondents had experienced three or fewer evaluations since the System had been introduced. The analysis of the responses indicated that there was great dissatisfaction with the current performance management system. A few of the reasons are that the system in place did not effectively measure the pharmacists performance, that additional work done was not recognised, and that the process was extremely time-consuming. It may be concluded that the government needs to address the current problems being experienced with this system, as at present, it is not meeting the objectives it was intended to meet.

- Description: The last decade has seen an exodus of South African nurses migrating to "greener pastures". As a result of this migration, the South African Healthcare Service has been drained of one of its most essential resources – nurses. Subsequently, the crippling flight of nurses has thrown the nursing profession into a state of crisis. The Kingdom of Saudi Arabia is one of the more popular destinations for South African nurses, the main reason being the attractive financial rewards. One agency reports that they send an average of thirty nurses a month to various hospitals within the Kingdom of Saudi Arabia. Saudi Arabia is an Islamic country. Due to the uniqueness of the enforcement of the Islamic faith and the Saudi culture, many restrictions are imposed, particularly on women. The challenges and problems facing the South African nurses were, therefore, unique compared to elsewhere in the world. This research study had a primary and a secondary objective: The primary objective of this study was to explore and describe the lived experiences of South African nurses related to living and working in Saudi Arabia; The secondary objective of this study was to develop orientation guidelines to support South African nurses working and living in Saudi Arabia. The researcher utilized a qualitative, explorative, descriptive and contextual design based on a phenomenological approach to inquiry, in an attempt to answer the question: "What are the professional and personal experiences of the South African registered nurses working and living in Saudi Arabia?" Eleven registered nurses were selected to participate by means of purposive sampling. These nurses had been living in Saudi Arabia between three and six months. Consent was obtained from participants and the ethics committee of both the Nelson Mandela Metropolitan University and King Faisal Specialist Hospital and Research Centre. The central theme emanating from the study was recognized as being 'Cultural Diversity'. The sub-themes identified related to the registered nurses’: - Religious/spiritual adaptation - Environmental adaptation - Emotional/psychological adaptation - Professional adaptation Based on the identified themes, guidelines were formulated to assist South African registered nurses when migrating to Saudi Arabia. Utilization of these should assist the South African registered nurse in assimilating into both the cultural and working environment. However, in reality, the outcome showed that no one can be prepared fully for what awaits them in Saudi Arabia.

The Kingdom of Saudi Arabia is one of the more popular destinations for South African nurses, the main reason being the attractive financial rewards. One agency reports that they send an average of thirty nurses a month to various hospitals within the Kingdom of Saudi Arabia. Saudi Arabia is an Islamic country. Due to the uniqueness of the enforcement of the Islamic faith and the Saudi culture, many restrictions are imposed, particularly on women. The challenges and problems facing the South African nurses were, therefore, unique compared to elsewhere in the world. This research study had a primary and a secondary objective: The primary objective of this study was to explore and describe the lived experiences of South African nurses related to living and working in Saudi Arabia; The secondary objective of this study was to develop orientation guidelines to support South African nurses working and living in Saudi Arabia. The researcher utilized a qualitative, explorative, descriptive and contextual design based on a phenomenological approach to inquiry, in an attempt to answer the question: "What are the professional and personal experiences of the South African registered nurses working and living in Saudi Arabia?" Eleven registered nurses were selected to participate by means of purposive sampling. These nurses had been living in Saudi Arabia between three and six months. Consent was obtained from participants and the ethics committee of both the Nelson Mandela Metropolitan University and King Faisal Specialist Hospital and Research Centre. The central theme emanating from the study was recognized as being 'Cultural Diversity'. The sub-themes identified related to the registered nurses’: - Religious/spiritual adaptation - Environmental adaptation - Emotional/psychological adaptation - Professional adaptation Based on the identified themes, guidelines were formulated to assist South African registered nurses when migrating to Saudi Arabia. Utilization of these should assist the South African registered nurse in assimilating into both the cultural and working environment. However, in reality, the outcome showed that no one can be prepared fully for what awaits them in Saudi Arabia.

This increasing number of diabetics is expected to have enormous economical implications. One of the consequences of the increase in DM sufferers is the increase in deaths due to diabetes related complications. In 2000, 3.2 million people died of DM and diabetes related complications compared with 3 million who died from HIV/AIDS related illnesses (William & Pickup, 2004:64). Health services are financially burdened by DM complications that result in heavy demands in both direct costs of hospitalization and medication as well as indirect costs such as manpower of staff involved in the care of the patient (Leuner, 2000:410). Diabetic ketoacidosis (DKA) is a prominent life-threatening complication of type 1 diabetes mellitus and, if not treated medically, it can lead to coma and death. DKA accounts for 14 per cent of all diabetic related hospital admissions and has a mortality rate of 5 to 10 per cent (Pickup & Williams, 2003:1). Rapid identification of DKA is necessary to improve patient care. George- Gray and Chernecky (2002:96) suggest patient outcomes are affected by the nurse’s ability to recognize the clinical features of DKA. It is important, therefore, for the nurses to understand the signs and symptoms and treatment of this diabetes related complication. National health policies and treatment guidelines should be aimed at addressing DKA treatment through setting guidelines for diabetic care (Peters, 2004:22). The main goal of this study was to determine registered nurses’ knowledge of DKA and to develop guidelines to assist registered nurses in providing adequate and effective health care to prevent DKA reoccurrence. The research study is based pm Bergam’s model on quality of care and accountability. Bergman claims that several preconditions must be filled in order to be accountable, including: • Accountability • Responsibility • Authority • Ability (Bergman (1982:8) emphasizes that ability reflects the registered nurses’ accountability, which is composed of values, knowledge and skills.) The study was divided into two phases. Phase one determined registered nurses’ knowledge related to the management of DKA. Phase two of the study included the developing guidelines to assist registered nurses in providing adequate and effective health care to patients with DKA. The design of the study is quantitative, explorative, descriptive and contextual; a survey was performed using a questionnaire based on a literature study. Ethical principals were maintained throughout the study. Permission to the study was sought from the Local Health Authorities and the heads of department where the research was undertaken. Findings of the research were analysed and discussed in the research. The knowledge of the majority of the respondents was found to be average. Out of the thirty- two respondents 52.8 percent possessed adequate knowledge the management of DKA and 63.4 percent possessed adequate knowledge on the management of DM. Two recommendations to assist registered nurses in the implementation of guidelines of the recommendations were also provided.

Research has shown that such stressors can result in mental, physical and behavioural stress reactions among nurses (Demerouti, Bakker, Nachreiner and Schaufeli, 2000:454). The objective of this study is to explore and describe registered nurses’ perceptions of factors causing stress in intensive care environments of state hospitals. The main purpose of the study is to develop guidelines for a stress management programme, to assist registered nurses to cope with the stressors in an intensive care environment. The research design is placed within a quantitative, explorative and descriptive contextual framework. Validity and reliability in testing and evaluating the research questionnaire are discussed, as well as the ethical and legal considerations relating to this research study. - Description: The foundational principles of Holland’s (1985b, 1992, 1997) vocational theory state that career choice is an expression of personality, in that individuals with specific personality types seek out work or learning environments that match their personality types. Furthermore, interest inventories such as the Self-Directed Search (Holland, Powell, & Fritzsche, 1994) can be regarded as personality inventories and used to ensure that individuals find themselves in a working environment that is best suited to their interests and personality type. In addition to instruments that measure interests, Holland also developed dictionaries of occupations coded according to the characteristics and interests of different occupations and working environments, such as the Dictionary of Holland occupational codes (Gottfredson & Holland, 1996). The level of congruence between personality types and occupational types is linked to career satisfaction, stability and achievement. This premise only holds true, however, if the codes for the occupational environment suggested by Holland (1985c) are, in fact, valid. In other words, do the codes suggested by Holland match the personality types of individuals entering or who are already employed in a particular work or learning environment? There have been many studies conducted with regard to the validity of Holland’s prescribed interest typology codes. Spokane, Meier and Catalano (2000) located a large number of empirical studies published since 1985 that relate directly to the validity of Holland’s codes, many of which have suggested that the existing codes may not be valid. There is a scarcity of such research in South Africa, especially with regard to vital and growing occupational fields, such as the ecotourism industry, that are so important to the economic well-being of the country. Consequently, the overall aim of the present study was to explore and describe the interest codes of male and female South African student and working field guides in order to discuss the appropriateness of the occupational codes prescribed by Holland for the field guiding profession. The quantitative design of the study was exploratory-descriptive in nature and made use of the Self-Directed Search Questionnaire (Holland, Fritzsche, & Powell, 1994). The total sample consisted of 100 participants (68 male and 32 female), constituting 40 working field guides and 60 field guide students. Descriptive analyses were conducted with the use of frequency tables, charts, means and standard ix deviations. In addition, Independent sample t-tests were computed in order to describe and compare the mean scores of the interests for males and females, and to compare the mean scores of the interests for student and working field guides. The results of this study suggest that the prescribed occupational code (SRI; Social, Realistic, Investigative) for South African field guides may not be appropriate and that an interest profile such as SREI AC may be more appropriate. These results, in combination with the findings of the research review, suggest the need for a more extensive, cross-cultural study to determine the appropriateness of Holland’s occupational codes for South African occupations, which may lead to a revision of the South African Dictionary of Occupations (Taljaard & von Mollendorf, 1987).

- Description: Career psychology in South Africa has traditionally been constituted by the vocabularies, assessment methods, counselling practices and research objectives of the modernist-positivist paradigm. This paradigm has produced a rich but disparate and fragmented range of career theories, research perspectives and career education practices that have been limited in their consideration and integration of the broad range of contextual factors that influence the career development of South African adolescents in unique ways. This limitation has had, and still has, the potential of promoting prescriptive and disqualifying constructions of career development for South African youth. A search for alternatives to traditional modernist-positivist understandings of career has led, however, to a further fragmentation of the career field into what can broadly be termed qualitative and quantitative approaches. This twofold fragmentation, as well as the dynamic complexity of the world of work in the twenty-first century, has inspired this study’s investigation of an integrating framework that employs a wide range of career theoretical perspectives in the service of constructing experience-near accounts of the complex and fluid interrelationship between individual career makers and their specific social, environmental and societal contexts. The present study has therefore employed the Systems Theory Framework (STF) in investigating and co-constructing representations of the career development of a group of South African adolescents in a way that acknowledges their unique systems of career influence and discursive contexts. The research adopted an exploratory-descriptive design in collaborating with the participants in this investigation. In the first phase of the study a sample of 70 grade 11 male and female adolescents from middle socioeconomic status environments were invited to complete the My Systems of Career Influences (MSCI) workbook in re-presenting systemic constructions of their career development. Tesch’s model of qualitative content analysis and frequency counts has been used to re-present that process to you in this text. In the second phase of the study the researcher collaborated with one participant in a systemic narrative career counselling process. During this process an account of the participant’s career narrative was co-constructed in conversations guided by a poststructural narrative approach to career counselling and the MSCI’s structuring of the participant’s complex systems of influence. The co-constructed account was critically examined according to Parker’s approach to discourse analysis. The second phase investigated how the counselling and research processes had positioned the participant in relation to her influential systems and their privileged discourses of career development. The study is particularly pertinent to a growing need for the development of respectful, critical and non-discriminatory career assessment, career research and career counselling collaborations between professionals and career makers navigating the unique and diverse South African context.
